Qualifications: Previous licensure as an Ohio Nursing Home Administrator from 1980 to the present and license expired more than 12 months ago.

If you were previously licensed in Ohio since 1980, you meet Ohio’s current minimum qualifications.  You will have to submit an application for Board review.  After approval you must pass the National, Core and State exams.  To begin this process, email jsee@age.ohio.gov with the following subject: “Relicense Application Request.” Include the following information:

1. Your name;
2. Mailing Address;
3. Your former license number;
4. Telephone numbers where you can be reached;
5. If your name has changed, please state your previous name.
